The AzPA Pain Management Certificate Program (PMCP) is an innovative and comprehensive learning activity developed to assist pharmacists to maintain and enhance their professional competencies in pain management.  Upon completion of the PMCP, pharmacists will be able to ensure optimal medication therapy outcomes and patient safety, recommend nonpharmacologic and pharmacologic therapies, provide medication management in various health care settings, develop new roles and responsibilities for a pharmacist, and obtain educational requirements for pharmacist re-licensure. 

Course Modules: The purpose of this certificate program is to provide comprehensive education and skills to recommend treatment plans for pain management: Specific learning objectives are listed within each learning module.

Arizona Pharmacy Association Pain Management Program is conducted in two parts: the self-study readings and learning exercises, and one live webinar training session on the pharmacist's roles in chronic pain management and how to communicate with patients and providers about issues related to common pain syndromes, monitoring, and treatment approaches. A Certificate of Achievement (in PDF format) will be awarded to participants who successfully complete ten home-study modules, participate in the live webinar and pass an examination that is applicable to the practice of pharmacy.  Electronic Certificates will be issued within 30 days of completion of this certificate training program.

Module 1:  Pain Management and Opioid Guidelines
 
At the completion of this home-study module, the participant should be able to:
  1. Describe the characteristics, clinical presentation, assessment, and desired outcomes of acute, chronic, and cancer pain.
  2. Recognize non-pharmacologic therapies used in the management of pain.
  3. Discuss pharmacologic treatment approaches (e.g., non-opioid analgesics, opioid analgesics, central analgesics, opioid antagonists), dosing guidelines, and an algorithm for acute pain. 
  4. Recommend when personalized pharmacotherapy (e.g. genotyping) is helpful in the treatment of pain.
  5. List monitoring parameters for evaluating therapeutic outcome and adverse effects from pain medications.
  6. Describe treatment approaches for mild, mild-moderate, and moderate-severe pain for oncology patients. 
  7. Discuss the 2016 CDC guidelines for prescribing opioids for chronic pain that includes the key recommendations when prescribing, selecting, dosing, monitoring for adverse effects and overuse, and discontinuing opioids. 
  8. Identify non-opioid therapies that can substitute for opioids when treating chronic pain. 

   

   

Module 2:  Pain Assessment
 
At the completion of this activity, the participant should be able to:
  1. Identify the psychometric properties of pain-assessment tools and determine their role in evaluating the quality of an assessment tool.
  2. Review the different types of pain-assessment tools and the general principles for using a pain-assessment tool.
  3. Identify pain-assessment tools validated in special patient populations and select an appropriate pain-assessment tool for a specific patient.
  4. Compare and contrast the advantages and disadvantages of unidimensional, multidimensional, and observational/behavioral pain-assessment tools. 
  5. List what needs to be assessed or monitored to effectively care for a patient in pain.
 

   

   

Module 3: Drug-Drug Interactions and Pharmacogenetics
 
After completing this activity, the participant should be able to:
  1. Identify the mechanisms of action associated with analgesic drug interactions, especially those involving the CYP450 enzyme system.
  2. Recognize the significance of pain medication drug interactions with respect to morbidity and mortality.
  3. List major drug interactions associated with pain relievers.
  4. Develop strategies to avoid or minimize potential analgesic drug interactions.
  5. Recommend evidence-based guidelines for interpreting genetic information and making clinical recommendations. 
  6. Provide information to patients on select opioids that are metabolized by CYP450 enzymes and recommend when genetic testing may be useful. 

   

   

Module 4:  Neuropathic Pain and Fibromyalgia
 
 
At the completion of this activity, the participant should be able to:
  1. Describe the pathophysiology of neuropathic pain syndromes.
  2. Explain the mechanism for the development of painful diabetic peripheral neuropathy, HIV/AIDS-neuropathy, and post-herpetic neuralgia.
  3. Specify risk factors for painful diabetic peripheral neuropathy, HIV/AIDS-neuropathy, and post-herpetic neuralgia. 
  4. Review the current literature on the management of painful diabetic peripheral neuropathy, HIV/AIDS-neuropathy, and post-herpetic neuralgia.
  5. Differentiate between treatment strategies for the management of painful diabetic peripheral neuropathy, HIV/AIDS-neuropathy, and post-herpetic neuralgia. 6. List benefits of the use of topical analgesic formulations in the management of neuropathic pain.
  6. List benefits of the use of topical analgesic formulations in the management of neuropathic pain.
  7. Identify patient populations with neuropathic pain that may benefit from the use of topical analgesics.
  8. Discuss the pathogenesis, diagnosis, and treatment of fibromyalgia.
  9. Assess the use of evidence-based medicine in the management of neuropathic low back and neck pain.
  10. Apply knowledge learned in managing the patient with neuropathic pain secondary to diabetic peripheral neuropathy, HIV/AIDS, post-herpetic neuralgia, fibromyalgia, and low back/neck pain.

   

   

Module 5:  Headache
 

At the completion of this home-study module, the participant should be able to:

  1. Recognize the International Headache Society (HIS) classification for diagnosing different types of headaches (Table 61-1 and 61-2).
  2. Describe the clinical presentation (signs and symptoms), common triggers, laboratory tests, diagnostic tests, and general treatment approaches for migraine headaches.
  3. Recommend dosing guidelines and treatment algorithms of acute migraine and prophylactic migraine therapies (Table 61-4 and 61-5;Figures 61-2 and 61-3). 
  4. Compare and contrast pharmacologic agents used in the management of acute migraines (e.g., analgesics, NSAIDs, opiate analgesics, anti-emetics, ergot alkaloids and derivatives, and serotonin receptor agonists).   
  5. Discuss pharmacologic agents used in the prophylactic management of migraine headaches (e.g., beta-adrenergic antagonists, antidepressants, anticonvulsants, NSAIDs, triptans). 
  6. Describe the epidemiology, pathophysiology, clinical presentation, and most common approaches for the treatment of tension-type headaches (non-pharmacologic and pharmacologic).
  7. Discuss the epidemiology, pathophysiology, clinical presentation, and treatment approaches for cluster headaches (abortive therapy and prophylactic therapy). 

   

   

Module 6:  Pain Conditions:  Sickle Cell, Cancer, Arthritis, and Postoperative
 

At the completion of this home-study module, the participant should be able to:

  1. Describe the pathophysiology of sickle cell anemia-related pain, cancer pain, and postoperative pain.
  2. Specify risk factors for sickle cell pain crises, cancer-related pain, chronic pain in cancer survivors, and in pain due to rheumatoid arthritis. 
  3. Review the current literature on the management of pain in people with sickle cell anemia, cancer, and rheumatoid arthritis.
  4. Discuss the current clinical practice guidelines for the use of opioids for persons with chronic pain, individuals after surgery, and in critically-ill people.

   

   

Module 7:  Palliative and Hospice Care
 

At the completion of this home-study module, the participant should be able to:

  1. Describe potential roles and responsibilities for a pharmacist working in palliative and hospice care.
  2. List examples of tasks, skills, and knowledge for pharmacists in palliative and hospice care.
  3. Recognize journals, books, websites, and organizations for professional development of palliative and hospice care pharmacists.

   

   

Module 8:  Complementary Pain Approaches:  Chronic Pain of Neck, Back, Knees
 

At the completion of this home-study module, the participant should be able to:

  1. Review the prevalence of complementary health approaches for pain management in the United States.
  2. Define complementary health approach.
  3. Select the most appropriate complementary health approach for a specific patient condition with back, knee, or neck pain.
  4. Assess the current evidence supporting the use of complementary health approaches for pain management.
  5. Identify the pharmacologic treatment options for patients with back pain.
  6. Discuss when a referral to a specialist in back pain is warranted.
 
 

   

   

Module 9:  Special Populations: Elderly, Pediatric, and Pregnancy 
 

At the completion of this home-study module, the participant should be able to:

      Elderly
  1. Describe age-related changes in renal, gastrointestinal, and hepatic function, and changes in the brain and central nervous system.
  2. Explain how age-related changes in physiological function can affect the pharmacokinetics and pharmacodynamics of pain management in the older adult.
  3. Specify the impact of pain and its management on the following conditions in older adults: heart failure, end stage renal disease, dementia, frailty, cancer, bone pain, and neuropathic pain.
  4. Review the barriers to the management of persistent pain in older adults.
  5. Differentiate between pharmacological and nonpharmacological therapies that have been found to be beneficial in the management of persistent pain in older adults.
  6. List the core elements that are involved in building a therapeutic alliance in managing pain in the older adult.
  7. Identify the step-wise approach to managing neuropathic pain in the older adult.
  8. Discuss the clinical studies that have investigated the efficacy of analgesic treatment on pain intensity/behavior in advanced dementia.
  9. Assess the ability of pain scales to identify and monitor pain in patients with advanced dementia.
  10. Apply knowledge of the renal effects of over-the-counter analgesics when counseling the older adult. 
     Pediatric Pain Management
  1. Recognize common dosing errors for pediatric patients.
  2. Evaluate the effectiveness and safety of commonly used analgesics for certain indications.
  3. Discuss recent FDA Drug Safety Communications regarding the risks of using codeine and tramadol in pediatric patients.
  4. Recommend adverse event monitoring for families with children using analgesics.
     Pregnancy Pain Management
  1. Identify websites that contain up to date information about the benefits and risks of taking medications during pregnancy and lactation.
  2. Discuss possible risks to the pregnancy when opioid pain medications are used.
  3. Provide patient education regarding use of taking opioids during pregnancy and opioid use disorder treatment.
 

   

   

Module 10:  Opioid-Use Disorder
After completing this activity, the participant should be able to:
  1. Recognize the criteria for opioid-use disorder according to the Diagnostic and Statistical Manual of Mental Disorders, fifth edition.
  2. Describe the signs and symptoms of opioid withdrawal (e.g., physical symptoms, general pain, and psychological symptoms).
  3. Recommend treatment approaches for opioid withdrawal (e.g., alpha-2 adrenergic agonists, non-opioid agents, and opioid agents).
  4. Discuss the use of naltrexone for abstinence treatment from opioids.
  5. Compare and contrast the use of methadone and buprenorphine for maintenance approaches for opioid use disorder.
  6. List approaches for how pharmacists can help in the prevention and treatment efforts of opioid use disorder and overdose.

At the conclusion of this activity, the participant will be able to:
  1. Discuss the IOM’s Relieving Pain in America, the National Pain Strategy, the CDC Guideline for Prescribing Opioids for Chronic Pain, and the NASEM panel’s recommendation for mandatory education on opioid use disorders and pain treatment for prescribers, pharmacists, and others in healthcare.
  2. Recognize the characteristics and signs of opioid use disorder and methods for treatment, including deprescribing, tapering opioids for chronic pain conditions, and maintenance treatments (e.g., methadone, buprenorphine, and naloxone).
  3. Outline how pharmacists can be involved with providing appropriate and safe pain management, recommending opiate conversion strategies, helping to curb the opioid epidemic, participating in reducing opioid overdose (e.g., dispensing of naloxone), and promoting evidence-based treatments for opioid use disorders.
  4. Discuss the risk/benefit of medical marijuana, cannabis drug-drug interactions, and when it may be appropriate for pain management.
  5. Recommend nonpharmacologic and nonopioid pharmacologic treatment approaches for various pain conditions.

Brief Bio : Carrie Vogler, Pharm.D., BCPS is a Clinical Associate Professor with the Department of Pharmacy Practice at Southern Illinois University Edwardsville School of Pharmacy. She also holds a joint appointment with the Southern Illinois University School of Medicine, Division of General Internal Medicine at Memorial Medical Center in Springfield, Illinois. She participates in daily multi-disciplinary, inpatient teaching rounds with the internal medicine teams. She is a preceptor for the Pharmacy Practice Residency program at her institution. She joined the Southern Illinois University Controlled Substances Work Group and helped create patient education videos on chronic pain management topics. She helped update the clinic policy and procedures for controlled substances. She developed and taught pain education classes to patients that suffer from chronic pain. She currently chairs the School of Pharmacy Faculty Development Committee and coordinates a Physical Assessment and Patient Care Skills Course and an Acute Care Pharmacotherapy Elective. She serves as Director of Marketing Affairs for the Illinois Council of Health-System Pharmacists. Dr. Vogler received her doctorate of pharmacy from Midwestern University Chicago College of Pharmacy and completed her pharmacy practice residency at Saint Louis University Hospital. Dr. Vogler’s areas of interest include pain management, patient education, and medication reconciliation.

Brief Bio : Dr. Donna Lisi is a board-certified psychiatric pharmacist and pharmacotherapy specialist. Dr. Lisi earned both her Bachelor of Science in Pharmacy and Doctor of Pharmacy degrees from St. John's University in New York. She then went on to complete a 2-1/2 year post-doctoral fellowship in Geriatric Pharmacy, which was sponsored by the National Institute on Aging. Dr. Lisi has held appointments at several colleges of pharmacy throughout the country, including Philadelphia College of Pharmacy. She was Clinical Pharmacist in Geriatrics at the NJ VA Health Care System for 7 years and served as Corporate Drug Information Specialist for Barnabas Health Care System. Recently, Dr. Lisi completed the Geriatric Education Center's Faculty Development Collaborative Scholars Program. She is Medication Therapy Management certified and is a Certified Dementia Practitioner. For 16 years, she developed clinical practice guidelines that were used in long term care facilities throughout the country. Currently, Dr. Lisi is a freelance medical writer and educator. She has authored numerous article on pain management. She also serves on the Somerset County Local Advisory Council on Alcohol and Drug Abuse, Somerset County Aging Advisory Council and is an active participant at her local township Board of Health Advisory Council.

Brief Bio : Dr. Kelly Mendoza is a Pain Management Clinical Pharmacist at Kaweah Delta Health Care District in Visalia, California. Dr. Mendoza graduated from Midwestern University, College of Pharmacy-Glendale in Glendale, AZ with her Doctor of Pharmacy degree in 2015 and then completed her PGY-1 Pharmacy Residency at Banner Estrella Medical Center in Phoenix, AZ. She completed a PGY-2 Pain Management/Palliative Care Pharmacy Residency at The University of Maryland School of Pharmacy in Baltimore, MD. Dr. Mendoza is an active member of the American Society of Health-System Pharmacists, American Pharmacists Association, Society of Palliative Care Pharmacists, American College of Clinical Pharmacy, and Phi Delta Chi. In the future, Dr. Mendoza plans on becoming board certified in pharmacotherapy.

Brief Bio : Martha P. Fankhauser, B.A. Biology, B.S. Pharmacy, M.S. Pharmacy, FASHP, FAzPA, and Board Certified in Psychiatric Pharmacy (BCPP) specializes in Neuropsychiatric Pharmacy Practice. She completed a neuropsychiatric pharmacy residency program and was a clinical professor from 1982 to 2010 in the Department of Pharmacy Practice at the University of Arizona College of Pharmacy, Tucson, Arizona. She has served as a Pharmacotherapy Specialist for Arizona Smokers ’ Helpline at the Mel and Enid Zuckerman College of Public Health. She is actively involved in promoting psychiatric pharmacy practice through the Arizona Pharmacy Association and helped develop a psychiatric certificate program and a pain management certificate program for pharmacists. Since 2010, she has a private consulting practice and provides guidance and recommendations for individuals who want an assessment of their medications, over-the-counter products, and complementary medicine treatments (www.martiefankhauser.com). Ms. Fankhauser is currently a Medication Management Specialist at SinfoníaRx and works at the Assurance Health and Wellness Center in Tucson, Arizona. Her interests are in preventative health/wellness interventions, polypharmacy and drug interactions, substance use disorders, tobacco cessation, pain management, and nutritional deficiencies.
