APhA’s The Pharmacist & Patient-Centered Diabetes Care certificate training program (6th Edition) is an intensive educational experience designed to equip pharmacists with the knowledge, skills, and confidence needed to provide effective, evidence-based diabetes care. Five self-study modules provide comprehensive instruction in current diabetes concepts and standards of care. The live seminar incorporates case studies and hands-on skills training focused on the situations most likely to be encountered—as well as the services most needed—in community and ambulatory care practice settings. Participants will gain experience evaluating and adjusting drug therapy regimens for patients with type 1 and type 2 diabetes, counseling patients about lifestyle interventions, analyzing and interpreting self-monitoring of blood glucose results, and assessing the overall health status of patients to identify needed monitoring and interventions.
The goals of this program are to:
- Provide comprehensive instruction on current standards of care for patients with diabetes
- Increase pharmacists’ ability to apply pharmacotherapeutic information and serve as the drug therapy expert on the diabetes health care team
- Refresh pharmacists’ knowledge of the pathophysiology of diabetes and the acute and long?term complications of the disease
- Familiarize pharmacists with important concepts in nutrition, exercise, and weight control that contribute to optimal diabetes care
- Provide training on the use of diabetes-related devices and physical assessments involved with optimal diabetes care
- Describe business opportunities and roles for pharmacists in improving health outcomes for patients with diabetes
APhA’s The Pharmacist & Patient-Centered Diabetes Care certificate training program has two components: online interactive self-study with assessment; the live seminar which includes demonstrating competency in blood pressure testing, self-injection techniques, diabetic foot exam, and blood glucose testing, followed up with online evaluation and assessment. A Certificate of Achievement will be awarded to participants who successfully complete all program components.
Key Learning Objectives of the Live Training Seminar:
- Evaluate the overall health status of patients with diabetes in terms of recommended monitoring and interventions, and formulate strategies for closing gaps in care.
- Propose modifications to a patient’s drug therapy regimen rooted in evidence-based algorithms for diabetes management
- Recommend dietary interventions to support optimal glycemic control and weight loss (when indicated) in patients with diabetes
- Analyze and interpret a patient’s self-monitoring of blood glucose results and use the results to identify needed changes in the diabetes management plan
- Demonstrate proper technique for measuring blood pressure, administering injections, obtaining fingerstick samples for blood glucose monitoring, operating blood glucose meters, and performing monofilament foot testing
- Integrate the varied aspects of comprehensive diabetes care into efficient, sensitive, respectful pharmacist-patient interactions that support optimal patient self-management

The self-study learning portion of Pharmacist and Patient-Centered Diabetes Care Certificate Training Program is approved for 15 hours (1.5 CEU) of continuing pharmacy education credit (UAN ACPE UAN: 0202-9999-18-121-H01-P). The seminar is approved for 8 hours (0.8 CEU) of continuing pharmacy education credit (UAN: 0202-9999-18-122-L01-P)
- Successful completion of the self-study component involves passing the online self-study assessment with a grade of 70% or higher and will result in 15 contact hours of CPE credit (1.5 CEUs). ACPE Universal Activity Number: 0202-9999-18-121-H01-P. SELF-STUDY MUST BE COMPLETED PRIOR TO THE LIVE SESSION.
- Successful completion of the live seminar component involves attending the full live seminar, demonstrating competency in blood pressure testing, self-injection techniques, diabetic foot exam, and blood glucose testing, passing the online final exam with a grade of 70% or higher, as well as, completing the online evaluation. Successful completion of this component will result in 8 contact hours of CPE credit (0.8 CEU). ACPE Universal Activity Number: 0202-9999-18-122-L01-P
Once credit is claimed, Statements of Credit will be available online within 24 hours on participant’s CPE Monitor profile at www.nabp.net. The Certificate of Achievement will be available online upon successful completion of the necessary activity requirements on the participant’s "My Training” page on www.pharmacist.com. Darren Clonts, PharmD, MBA
Brief Bio : Darren Clonts is a Clinical Pharmacist with Cigna Medical Group (CMG) and actively participates in the management of diabetic patients via a collaborative practice arrangement, Darren began his work with Cigna in 2009 and has held various roles including Pharmacist, Pharmacy Supervisor, and other project management and leadership oriented roles. Darren received his Doctor of Pharmacy degree from the University of Arizona College of Pharmacy in 2009. In 2015, he received his Master in Business Administration degree from the University of Phoenix School of Business. Darren also holds Certificates in Medication Therapy Management and Diabetes Care from the American Pharmacist Association. Darren is an active member of the Arizona Pharmacy Association and currently sits on their Board of Directors representing Managed Care Pharmacy. Darren's interests include collaborative practice arrangements, process improvement (six sigma), accountable care organizations (ACO), Medicare Advantage and other value based payment models and how pharmacists fit into the equation. Darren resides with his wife and 4 children in Gilbert, Arizona.
Whitney Rice, PharmD, BCACP
Brief Bio : Whitney Rice is a graduate of Midwestern University College of Pharmacy Glendale and currently serves as the Clinical Sales Manager for Fry’s Pharmacy. She started her pharmacy career at Fry’s, providing pharmacy clinical services such as immunizations, MTM, and disease state management programs. Whitney is a Board Certified Ambulatory Care Pharmacist, PGY1 Residency Program Director, and Adjunct Assistant Professor at Midwestern University College of Pharmacy Glendale. She is an active member and fellow of AzPA, and currently serves on the Board of Directors as Secretary.
